Make home visits to clients in designated geographic territories.
Conduct evaluations to diagnose and treat speech, language, or swallowing issues.
Develop treatment plans based on evaluations, orders, and goals.
Update and revise care plans with the multidisciplinary team.
Educate patients and families on rehabilitative care for independence.
Document observations and interventions using a touchpad EMR.

🎯 Requirements

Active PA license as Speech Language Pathologist.
CCC-SLP from ASHA or equivalent qualifications.
Strong computer skills; EMR experience preferred.
Prior home health experience is a plus.
