Make home visits to clients in designated geographic territories.
Perform evaluations to assess and treat speech, language, or swallowing difficulties.
Develop treatment plans from evaluations, orders, and goals.
Assess and revise care plans with the multidisciplinary team.
Educate patients and families on rehabilitative care and safety.
Document care observations using a touchpad tablet.

🎯 Requirements

Current Colorado license.
ASHA CCC or equivalent certification in progress.
Meet equivalent education and work experience toward certification.
Complete education and supervised experience toward certification.
Solid computer skills; EMR experience preferred.
