- Make home visits to clients in designated geographic territories.
- Perform evaluations to assess and treat speech, language, or swallowing issues.
- Develop a treatment plan based on evaluation, orders, and goals.
- Continually assess and revise the care plan; participate with multidisciplinary team.
- Educate patients and families in rehabilitative care for health, safety, independence.
- Document observations and interventions using a touch pad tablet.
🎯 Requirements
- Current North Carolina Speech Language Pathologist license.
- ASHA CCC-SLP or equivalent for certification; or awaiting exam.
- Complete education/work requirements for certification.
- Solid computer skills; EMR experience preferred.
- Prior home care experience as SLP is a plus.
