Make home visits to clients in designated geographic territories.
Perform evaluations to assess and treat speech, language, or swallowing difficulties.
Develop a treatment plan based on evaluation and orders.
Continuously assess and revise the speech therapy plan with the multidisciplinary team.
Educate patients and families in rehabilitative care to promote health and independence.
Document observations and care using a touch pad tablet.

🎯 Requirements

A current license as a Speech Language Pathologist in North Carolina.
Strong computer skills; EMR experience preferred.
