Make home visits to clients in designated territories.
Perform evaluations to assess and treat speech, language, or swallowing disorders.
Develop treatment plans based on evaluations and physician orders.
Continuously assess and revise care plan with the multidisciplinary team.
Educate patients and families in rehabilitation and safety for independence.
Document observations and interventions using a touchpad tablet.

🎯 Requirements

Active license as a Speech Language Pathologist in North Carolina.
ASHA CCC-SLP or equivalent.
Equivalent education for certification; awaiting ASHA exam.
In progress toward supervised experience for certification.
Solid computer skills; EMR experience preferred.
