Make home visits to clients in designated geographic territories.
Perform evaluations to diagnose and treat speech, language, or swallowing issues.
Develop a treatment plan based on evaluations, orders, and goals.
Continuously assess and revise the care plan with BAYADA clinicians in the multidisciplinary team.
Educate patients and families in rehabilitative care to promote independence.
Document observations and interventions using a touch pad tablet.

🎯 Requirements

Current North Carolina SLP license.
Proficient with computers; EMR experience preferred.
Prior home health SLP experience required.
