Make home visits to clients in designated territories.
Evaluate, diagnose, and treat speech, language, and swallowing issues.
Develop treatment plans based on evaluations and goals.
Reassess and revise care plans with the multidisciplinary team.
Educate patients and families on rehabilitative care for safety and independence.
Document observations and interventions in the EMR on a touch tablet.

🎯 Requirements

Current Vermont license as a Speech-Language Pathologist.
ASHA CCC in Speech-Language Pathology.
Educational requirements met; eligible for ASHA exam.
In progress with supervised experience toward certification.
Strong computer skills; EMR experience preferred.
