- Make home visits to clients in designated geographic territories.
- Perform evaluations to assess, diagnose, and treat speech, language, or swallowing issues.
- Develop a treatment plan based on evaluation, physician orders, and goals.
- Continuously assess and revise the care plan; participate in multidisciplinary team efforts.
- Educate patients and families on rehabilitative care to promote health and independence.
- Document care observations using a tablet during visits.
🎯 Requirements
- A current Ohio license as a Speech Language Pathologist.
- ASHA CCC-SLP or equivalent; or pursuing certification.
- Completion of education and supervised experience for certification.
- Solid computer skills; EMR experience preferred.
- Ability to collaborate with a multidisciplinary care team.
- Availability for per diem home health visits.
