Speech Language Pathologist - Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform evaluations to assess, diagnose, and treat speech, language, or swallowing issues.
• Develop a treatment plan based on evaluation, physician's orders, and goals.
• Continually assess and revise care plans; participate in multidisciplinary care.
• Educate patients and families in rehabilitative care to promote health and independence.
• Document observations and interventions using a touch pad tablet.

🎯 Requirements

• A current license as a Speech Language Pathologist in Maryland.
• ASHA CCC-SLP certification, or equivalent.
• Equivalent education and work experience toward certification, and awaiting ASHA exam.
• Completion of education and supervised work experience toward certification.
• Solid computer skills; EMR experience preferred.
