Speech Language Pathologist - Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform evaluations for speech, language, or swallowing issues.
• Develop and update treatment plans per orders and goals.
• Collaborate with BAYADA clinicians in the multidisciplinary care plan.
• Educate patients and families on rehabilitative care at home.
• Document observations and interventions using a tablet EMR.

🎯 Requirements

• Current PA license as Speech-Language Pathologist.
• ASHA CCC-SLP or eligible for certification.
• Meet education and experience requirements for certification.
• Solid computer skills; EMR experience preferred.
• Able to work in home health and with a multidisciplinary team.
• Willing to travel within designated territories.
