Make home visits to clients in designated territories.
Perform evaluations to diagnose and treat speech, language, swallowing issues.
Develop treatment plan based on evaluation, orders, and goals.
Assess and revise the speech therapy care plan; participate with the multidisciplinary team.
Educate and instruct patients and families in rehab and activities.
Document observations and interventions using a state-of-the-art touch pad tablet.

🎯 Requirements

Current PA license as Speech Language Pathologist.
ASHA CCC-SLP or equivalent certification path.
In process to take ASHA exam or accumulating supervised experience.
Solid computer skills; EMR experience preferred.
Prior home care experience a plus, not required.
