Speech Language Pathologist - Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform evaluations to diagnose and treat speech, language, or swallowing issues.
• Develop a treatment plan based on evaluation, orders, and goals.
• Continuously assess and revise the care plan with the multidisciplinary team.
• Educate patients and families in rehabilitative care to promote health, safety, and independence.
• Document observations and interventions using a touchpad tablet.

🎯 Requirements

• Current Maryland license as Speech-Language Pathologist.
• ASHA CCC or equivalent; pursuing the ASHA exam or certification as applicable.
• Completed education requirements and working toward supervised experience for certification.
• Solid computer skills; EMR experience preferred.
