Speech Language Pathologist, Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform evaluations to assess and treat speech, language, or swallowing difficulties.
• Develop a treatment plan based on evaluations, orders, and goals.
• Continually assess and revise the care plan with BAYADA clinicians.
• Educate patients and families in rehabilitative care for health and independence.
• Document observations and interventions in the EMR touchpad.

🎯 Requirements

• VA license as Speech Language Pathologist.
• CCC-SLP or equivalent credentials; eligible to sit for the exam.
• Completed degree and supervised work experience toward certification.
• Prior home health experience strongly preferred.
• Solid computer skills; EMR experience preferred.
