Speech Language Pathologist, Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform evaluations to assess, diagnose, and provide therapy for speech, language, or swallowing issues.
• Develop a treatment plan based on evaluation, orders, and goals.
• Continuously assess and revise the care plan with BAYADA clinicians.
• Educate patients and families in rehabilitative care to promote health and independence.
• Document observations and interventions using a touch pad EMR tablet.

🎯 Requirements

• A current Vermont license as a Speech Language Pathologist.
• ASHA CCC certification or equivalent credentials (or in process toward certification).
• Be in the process of accumulating supervised experience for certification.
• Solid computer skills; EMR experience preferred.
