Make home visits to clients in designated territories.
Conduct evaluations to diagnose and treat speech, language, or swallowing difficulties.
Develop a treatment plan based on eval, orders, and goals.
Assess and revise care plans; participate in multidisciplinary care.
Educate patients and families in rehabilitative care for safety and independence.
Document observations and interventions using a touch pad tablet.

🎯 Requirements

VA licensed Speech-Language Pathologist.
ASHA CCC-SLP or equivalent.
Eligible for ASHA exam for certification.
Supervised experience for certification.
Solid computer skills; EMR experience preferred.
