📋 Description

• Make home visits to clients in designated geographic territories.
• Perform evaluations to assess and treat speech, language, or swallowing difficulties.
• Develop a treatment plan based on evaluation, orders, and goals.
• Continually assess and revise the speech therapy care plan; join the multidisciplinary team.
• Educate patients and families in rehabilitative care to promote health and independence.
• Accurately document observations, interventions, and evaluations using a touch pad tablet.

🎯 Requirements

• A current license as a Speech Language Pathologist in VA.
• CCC-SLP from ASHA, or equivalent.
• Equivalent education per transcript/diploma and experience required.
• Awaiting ASHA examination for certification, or in process.
• Solid computer skills; EMR experience preferred.
