- Make home visits to clients in designated geographic territories.
- Perform evaluations to assess, diagnose, and treat speech, language, or swallowing difficulties.
- Develop a treatment plan based on evaluation, orders, and goals.
- Continually assess and revise the care plan with the multidisciplinary team.
- Educate patients and families in rehabilitative care to promote health, safety, and independence.
- Document observations and interventions using a state-of-the-art touch pad tablet.
🎯 Requirements
- Current VA license as Speech Language Pathologist.
- ASHA CCC-SLP certification (or pursuing).
- Meet education requirements for certification, with transcript/diploma.
- In process of supervised work experience for certification.
- Solid computer skills; EMR experience preferred.
