Make home visits to clients in designated geographic territories.
Evaluate and treat clients with speech, language, or swallowing difficulties.
Develop a treatment plan based on evaluation, physician’s orders, and goals.
Continually assess and revise the speech therapy plan with the multidisciplinary care team.
Educate patients and families in rehabilitative care to promote independence.
Document care observations and interventions using a tablet.

🎯 Requirements

Current North Carolina Speech Language Pathologist license.
Solid computer skills; EMR experience preferred.
