Make home visits to clients in designated territories.
Perform evaluations to diagnose and treat speech, language, or swallowing issues.
Develop a treatment plan based on evaluation, orders, and goals.
Continually assess and revise the care plan; join multidisciplinary team.
Educate patients and families on rehabilitative care and activities.
Document observations and interventions using a tablet.

🎯 Requirements

NH Speech Language Pathologist license.
CCC-SLP from ASHA or equivalent.
Awaiting ASHA certification exam.
In process of required supervised experience for certification.
Solid computer skills; EMR experience preferred.
