Speech Language Pathologist, Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform evaluations to assess and treat speech, language, or swallowing issues.
• Develop a treatment plan based on eval, orders, and goals.
• Continuously assess and revise care plans with BAYADA clinicians.
• Educate patients and families on rehabilitative care for health and independence.
• Document observations and interventions using a tablet.

🎯 Requirements

• A current license as a Speech Language Pathologist in New Hampshire.
• CCC-SLP from ASHA, or equivalent.
• Equivalent education; be preparing to take the ASHA exam.
• In process of required supervised experience for certification.
• Solid computer skills; EMR experience preferred.
