Make home visits to clients in designated geographic territories.
Perform evaluations to assess, diagnose, and treat speech, language or swallowing issues.
Create treatment plans based on evals, orders, and goals.
Assess and revise care plans; collaborate in multidisciplinary BAYADA team.
Educate patients and families on rehab activities for safety and independence.
Document care using a touch-screen tablet.

🎯 Requirements

Current NC SLP license.
ASHA CCC or equivalent, or eligible for exam.
Relevant degree and certification requirements.
Strong computer skills; EMR experience preferred.
Ability to perform home health evaluations and planning.
