Speech Language Pathologist

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform evaluations to assess and diagnose speech, language, or swallowing issues.
• Develop a treatment plan based on evaluation, orders, and goals.
• Continually assess and revise the speech therapy care plan with the multidisciplinary team.
• Educate patients and families in rehabilitative care to promote safety and independence.
• Accurately document observations, interventions, and evaluations using a touch pad tablet.

🎯 Requirements

• Current Indiana license as Speech Language Pathologist.
• ASHA CCC or equivalent for certification, or pursuing the exam.
• Education requirements completed and pursuing supervised work experience for certification.
• Solid computer skills; EMR experience preferred.
