Provide in-home speech therapy evaluations and treatments for adults and geriatrics
Assess, diagnose, and treat speech, language, cognitive, and swallowing disorders
Develop individualized treatment plans with physicians and care teams
Modify care plans as patient needs change
Educate patients and caregivers to promote safety and independence
Complete timely documentation via tablet-based EMR

🎯 Requirements

Current NJ SLP license required
CCC-SLP (CCC-SLP) preferred
Home health experience preferred but not required
Strong communication and organizational skills
Comfortable with electronic documentation/EMR systems
