Make home visits to clients in designated territories.
Perform evaluations to diagnose and treat speech, language, or swallowing issues.
Develop a treatment plan based on evaluation, orders, and goals.
Continually assess and revise the care plan with the multidisciplinary team.
Educate patients and families to promote health, safety and independence.
Document observations and interventions in EMR via a touch pad tablet.

🎯 Requirements

Current AZ SLP license.
ASHA CCC or equivalent.
Graduate degree; eligible for ASHA certification.
Completed supervised experience for certification.
Strong computer skills; EMR experience preferred.
