Speech Language Pathologist

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform evaluations to diagnose and treat speech, language, or swallowing issues.
• Develop a treatment plan based on evaluation, orders, and goals.
• Assess and revise care plans; participate in multidisciplinary team.
• Educate patients and families in rehabilitative care.
• Document observations, interventions, and evaluations using a tablet.

🎯 Requirements

• Active VA license as Speech Language Pathologist
• ASHA CCC or equivalent certification/eligibility
• Equivalent education/work experience for ASHA certification
• Completed degree requirements or pursuing certification
• Solid computer skills; EMR experience preferred
• Prior home care experience a plus, not required
