Care Coordinator

📋 Description

• Provide primary support for clients seeking care to manage their mental and behavioral health wellness.
• Use a variety of technologies to connect clients with the appropriate therapist and maintain support through an episode of care.
• Use a consultative approach to identify the correct provider for care.
• Manage inbound and outbound calls and other forms of communication while routing messages to the care team.
• Interact with external partners and healthcare systems to ensure access to care.
• Conduct outreach to referred clients and provide onboarding services and assistance with getting into care.
