Create clear workflows and actionable metrics for performance.
Collaborate with clinical leaders, social workers, and cross-functional partners.
Translate social work practice into repeatable, executable playbooks and programs.

🎯 Requirements

MSW degree required.
Excellent written communication.
Experience translating social work into workflows or programs.
Care management models and outcomes reporting experience.
Familiar with EMRs EPIC and OCHIN.
Comfortable in fast-moving, ambiguous environments.
