Social Worker, MSW - Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Assist team in understanding social/emotional factors in client health.
• Provide clinical social evaluations and plan interventions.
• Identify community resource referrals for client and family needs.
• Advocate for clients and families navigating the community system.
• Contribute to the total plan of care with BAYADA clinicians.

🎯 Requirements

• MSW required; 1+ year community-based experience.
• CSWE accreditation or transcripts/degree.
• Ability to work independently and manage time.
• Strong interpersonal skills.
• Solid computer skills; EMR experience preferred.
• Ability to travel to cases as assigned.
