Social Worker - Compassionate Care Program

📋 Description

• Deliver patient-centered, value-based care for CKD/ESKD.
• Lead advanced care planning and goals-of-care discussions.
• Address social determinants of health and behavioral health needs.
• Facilitate hospice and palliative care referrals and transitions.
• Collaborate with nephrologists, nurses, and care coordinators.
• Fully remote; support patients across time zones Mon-Fri 8:30a-5p CST.

🎯 Requirements

• Master’s degree in Social Work (MSW)
• Active state social work licensure
• At least 2 years in a healthcare setting
• Strong advanced care planning and goals-of-care skills
• Excellent communication, counseling, and collaboration
• Proficient with MS Office, Outlook, and Teams
