Conduct psychosocial assessments and develop participant care plans with the IDT.
Deliver and document social work interventions and case management.
Coordinate care transitions with the PCP and IDT across settings.
Initiate and facilitate care conferences for coordinated care with participants and families.
Provide discharge planning when participants disenroll from the program.

🎯 Requirements

Master’s degree in social work (MSW) required.
Minimum 1 year experience with frail/elderly population preferred.
Experience coordinating care conference meetings preferred.
Experience with behavioral health or substance abuse issues preferred.
CPR and First Aid certification within 180 days of hire; certification preferred.
