Social Worker

📋 Description

• Act as an integral member of an IDT and collaborate with LCSWs
• Establish relationships with participants and develop care plans
• Conduct psychosocial assessments to determine needs, preferences, and goals
• Deliver and document social work interventions, including resources, care transitions, counseling, and case management
• Partner with the Primary Care Physician and IDT to guide smooth care transitions
• Initiate, coordinate, and facilitate care conference meetings

🎯 Requirements

• Master’s degree in social work (MSW) required
• Minimum of one year experience with frail or elderly population preferred
• Experience coordinating and facilitating care conference meetings preferred
• Experience assisting people with behavioral health or substance abuse issues preferred
• CPR and First Aid certification within 180 days of hire; maintain certification
