Social Worker

📋 Description

• Conduct psychosocial assessments with IDT to create care plans.
• Deliver and document social work interventions per care plans.
• Assist with resources, services, and case management.
• Partner with PCP and IDT to guide care transitions.
• Initiate and coordinate care conference meetings.
• Provide discharge planning when participants disenroll.

🎯 Requirements

• Master’s degree in social work (MSW) required.
• Minimum of one year of experience with frail or elderly population preferred.
• Experience coordinating and facilitating care conference meetings preferred.
• Experience assisting people with behavioral health or substance abuse issues preferred.
• CPR and First Aid certification within 180 days of hire; certification upon hire preferred.
