Social Worker

📋 Description

• Conduct psychosocial assessments to identify participants' needs and goals.
• Deliver and document social work interventions per care plans, coordinating resources.
• Partner with the Primary Care Physician and IDT to guide care transitions between settings.
• Initiate, coordinate, and facilitate care conference meetings for care coordination.
• Provide discharge planning when participants disenroll from the program.

🎯 Requirements

• Master’s degree in social work (MSW) required.
• Minimum of one year experience with frail or elderly population preferred.
• Experience coordinating and facilitating care conference meetings.
• Experience assisting people with behavioral health or substance abuse issues.
• CPR and First Aid certification within 180 days of hire.
• CPR/First Aid certification upon hire preferred.
