- Act as integral member of an IDT, not working in isolation
- Collaborate with LCSWs on complex behavioral health issues
- Establish deep relationships with participants on caseload
- Help participants age safely at home with community resources
- Coordinate care transitions between settings (hospital, SNF, home)
- LCSW supervision available after 6 months at no cost
🎯 Requirements
- MSW degree required
- 1 year+ experience with frail/elderly population preferred
- Experience coordinating care conferences preferred
- Experience with behavioral health or substance abuse preferred
- CPR and First Aid certification within 180 days of hire preferred
