Social Worker

📋 Description

• Conduct psychosocial assessments and develop participant care plans
• Deliver counseling and case management within an IDT
• Coordinate with Primary Care and IDT for smooth care transitions
• Initiate care conferences to ensure care coordination
• Provide discharge planning when participants disenroll
• Build deep relationships with participants and networks

🎯 Requirements

• MSW required (Master’s in Social Work)
• 1 year+ experience with frail/elderly preferred
• Experience coordinating care conference meetings preferred
• Experience with behavioral health or substance use issues preferred
• CPR/First Aid certification within 180 days of hire
• LCSW supervision after 6 months available
