Social Worker

📋 Description

• Conduct psychosocial assessments to identify needs, goals, and preferences.
• Deliver and document social work interventions per care plans.
• Coordinate with PCP and IDT to guide care transitions between settings.
• Initiate and facilitate care conferences with participants and networks.
• Provide discharge planning when participants disenroll from the program.
• Collaborate with IDT as an integral member, not an island.

🎯 Requirements

• MSW degree required.
• 1+ year experience with frail/elderly population preferred.
• Experience coordinating care conference meetings preferred.
• Experience with behavioral health or substance abuse issues preferred.
• CPR and First Aid certification within 180 days of hire; certification on hire preferred.
