Social Worker

📋 Description

• Conduct psychosocial assessments to identify needs, goals, and preferences.
• Participate in IDT meetings to develop participant care plans.
• Deliver and document social work interventions including resources, transitions, counseling, and case management.
• Partner with the Primary Care Physician and IDT for smooth care transitions between settings.
• Initiate and coordinate care conference meetings for coordination among the care team.
• Provide discharge planning when participants disenroll from the program.

🎯 Requirements

• Master’s degree in social work (MSW) required
• Minimum of one year of experience with frail/elderly population preferred
• Experience coordinating and facilitating care conference meetings preferred
• Experience assisting people with behavioral health or substance abuse issues preferred
• CPR and First Aid certification within 180 days of hire and maintained
