- Conduct psychosocial assessments to determine needs and goals
- Develop participant care plans with the IDT
- Deliver counseling, resources, and case management
- Coordinate care transitions with PCP and IDT
- Initiate and facilitate care conferences
- Provide discharge planning on disenrollment
🎯 Requirements
- Master’s degree in social work (MSW) required
- Minimum 1 year of frail/elderly population experience preferred
- Experience coordinating and facilitating care conferences preferred
- Experience assisting people with behavioral health or substance abuse issues preferred
- CPR and First Aid certification within 180 days of hire; certification at hire preferred
