- Act as an integral member of an IDT, not isolated

🎯 Requirements
- Master’s degree in social work (MSW) required
- Minimum 1 year experience with frail/elderly population preferred
- Experience coordinating and facilitating care conferences preferred
- Experience assisting people with behavioral health or substance issues preferred
- CPR and First Aid certification within 180 days of hire; maintain thereafter
