Social Worker

📋 Description

• Conduct psychosocial assessments to determine participant needs and goals.
• Deliver and document social work interventions per care plans.
• Partner with PCP and IDT to guide safe care transitions.
• Initiate, coordinate and facilitate care conference meetings.
• Provide discharge planning when participants disenroll.
• Build deep relationships with participants and their support networks.

🎯 Requirements

• MSW required.
• At least 1 year with frail/elderly preferred.
• Experience coordinating care conferences preferred.
• Experience assisting with behavioral health or substance issues preferred.
• CPR and First Aid certification within 180 days of hire.
• CPR and First Aid certification upon hire preferred.
