- Conduct psychosocial assessments and develop participant care plans
- Collaborate as an IDT member rather than working in isolation
- Establish deep relationships with participants on caseload
- Help participants age safely and independently in their homes with community resourcing
- Lead and participate in care conferences to coordinate care
- Provide discharge planning when participants disenroll from the program
🎯 Requirements
- MSW required (Master’s degree in social work)
- Minimum 1 year experience with frail or elderly population
- Experience coordinating and facilitating care conferences
- Experience assisting with behavioral health or substance abuse issues
- CPR and First Aid certification within 180 days of hire
- Maintain CPR/First Aid certification thereafter
