- Conduct psychosocial assessments and help develop participant care plans
- Deliver and document social work interventions, including resources and counseling
- Coordinate care transitions with PCP and IDT members
- Lead care conferences to coordinate care with participants and families
- Provide discharge planning when participants disenroll
🎯 Requirements
- MSW required
- 1+ year experience with frail/elderly population preferred
- Experience coordinating care conferences preferred
- Experience with behavioral health or substance use issues preferred
- CPR and First Aid certification within 180 days of hire preferred
