📋 Description
- Conduct psychosocial assessments and develop care plans with IDT.
- Deliver social work interventions: resources, care transitions, counseling.
- Partner with PCP and IDT for smooth care transitions.
- Initiate and facilitate care conferences to coordinate care.
- Provide discharge planning upon disenrollment.
- Collaborate with LCSWs on complex behavioral health issues.
🎯 Requirements
- Master’s degree in social work (MSW) required.
- Minimum 1 year with frail/elderly population preferred.
- Experience coordinating and facilitating care conferences preferred.
- Experience assisting people with behavioral health or substance abuse issues preferred.
- CPR and First Aid within 180 days of hire; maintain certification.
