- Establish rapport with participants and families to provide psychosocial support
- Deliver and document social work interventions and arrange resources and services
- In coordination with IDT, coordinate housing, DMV needs, utilities, and other community aid needs
- Complete IDT care plan reviews with participants and family members within regulatory time frames
- Conduct home visits, hospital visits, and skilled nursing visits as needed
- Assist with completing grievances, appeals incident reports, APS reports, and other quality and compliance requirements
🎯 Requirements
- High school diploma or equivalency
- Associate degree in social work, human services, social science, or relevant field preferred
- Reliable means of transportation
- Previous experience assisting people with behavioral health and substance abuse issues, preferred
- Previous experience facilitating solutions with difficult psychosocial and family circumstances preferred
- Must obtain CPR and First Aid within 180 days of hire and maintain certification
