- Establish rapport with participants and families for psychosocial support
- Deliver and document social work interventions; arrange resources and services
- Coordinate care with IDT on housing, DMV, utilities, and community aid
- Complete IDT care plan reviews with participants and families on time
- Conduct home, hospital, and skilled nursing visits as needed
- Assist with grievances, appeals, incident reports, and compliance tasks
🎯 Requirements
- High school diploma or equivalency
- Associate degree in social work or related field preferred
- Reliable transportation
- Experience assisting people with behavioral health and substance use issues, preferred
- Experience facilitating solutions with difficult psychosocial and family circumstances preferred
- CPR and First Aid certification within 180 days of hire; certification upon hire preferred
