- Establish rapport with participants and families for psychosocial support
- Deliver and document social work interventions; arrange resources
- Coordinate services with IDT; housing, DMV, utilities, community aid
- Complete IDT care plan reviews with participants and families on schedule
- Conduct home visits, hospital visits, skilled nursing visits as needed
- Assist with grievances, appeals, incident reports, and APS/quality tasks
🎯 Requirements
- High school diploma or equivalency
- Associate degree in social work or related field preferred
- Reliable means of transportation
- Experience assisting people with behavioral health and substance use issues, preferred
- Experience facilitating solutions for difficult psychosocial and family circumstances preferred
- CPR and First Aid certification within 180 days of hire (preferred)
