- Establish rapport with participants and families; provide psychosocial support
- Deliver and document social work interventions; arrange resources and services
- Coordinate services with the IDT for housing, DMV needs, utilities, and community aid
- Complete IDT care plan reviews with participants and families within regulatory time frames
- Conduct home visits, hospital visits, and skilled nursing visits as needed
- Assist with grievances, appeals, incident reports, APS and other quality/compliance tasks
🎯 Requirements
- High school diploma or equivalency
- Associate degree in social work, human services, social science, or related field preferred
- Reliable transportation
- Experience assisting people with behavioral health and substance issues preferred
- Experience facilitating solutions in difficult psychosocial/family circumstances preferred
- CPR and First Aid certification within 180 days of hire; certification on hire preferred
