- Establish rapport with participants and families to provide psychosocial support
- Deliver and document social work interventions within scope; arrange resources and aid care transitions
- Coordinate services with IDT for housing, DMV, utilities, and community aid
- Complete IDT care plan reviews with participants and families within regulatory time frames
- Conduct home visits, hospital visits, and skilled nursing visits as needed
- Assist with grievances, appeals, incident reports, APS reports, and other quality/compliance tasks
🎯 Requirements
- High school diploma or equivalency
- Associate degree in social work, human services, social science, or relevant field preferred
- Reliable means of transportation
- Previous experience assisting people with behavioral health and substance abuse issues, preferred
- Previous experience facilitating solutions with difficult psychosocial and family circumstances preferred
- Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred
