- Establish rapport with participants and families to provide psychosocial support
- Deliver and document social work interventions; arrange resources and services
- Coordinate services with IDT for housing, DMV needs, utilities, and other community aid
- Complete IDT care plan reviews with participants and families within regulatory timeframes
- Conduct home visits, hospital visits, and skilled nursing visits as needed
- Assist with grievances, appeals, incident and APS reports; quality/compliance tasks
🎯 Requirements
- High school diploma or equivalency
- Associate degree in social work, human services, social science, or relevant field preferred
- Reliable means of transportation
- Prior experience assisting people with behavioral health and substance abuse issues preferred
- Experience facilitating solutions with difficult psychosocial and family circumstances preferred
- CPR and First Aid certification within 180 days; hire certification preferred
