Social Services Supervisor

📋 Description

• Oversee Social Work team, Participant Advocates, and therapists.
• Lead daily operations delivering person-centered care.
• Collaborate with IDT to meet participants’ social needs.
• Advocate for participants and effective IDT communication.
• Engage with community/government resources for PACE.
• Uphold Habitat values and foster a joyful team culture.

🎯 Requirements

• Masters in Social Work (MSW) required.
• Licensed Clinical Social Worker (LCSW) preferred.
• Knowledge of PACE or similar healthcare operations.
• Two+ years supervisory experience in social services.
• Bilingual: Spanish preferred.
• Driver’s license and transportation required.
